9. Whether there are any tax warrants or tax liens against the property;
10. Whether there are any other legal obligations regarding the property;
11. Whether any adverse claims have been made;
12. Whether there are any easements;
13. Whether the owner has ever declared bankruptcy;
14. Whether there are any current tenants on the property; and
15. The signature and notarization.

This document can be completed by any person who owns a property and wishes to sell it. This document should only be completed by the owner of the property, as the owner is the only person who will have all the necessary knowledge about the state of the property.
Once it is filled out, the Affidavit must be notarized, which means the affiant signs before a Notary Public, after providing valid legal identification.
